How to fill offline Dummy form- Axis Mutual Fund
This is a common application form, for the first time investing in any fund, using this form.
1) write the distributor / agent * and employee unique identification number above the form. If you are not through a distributor / agent, please leave to column or write “Direct”.
(* if you invest through a distributor or agent, you will have to provide the agent register number (arn) and employee unique identification number of the form. The fund is provided by your portfolio information this agent (ARN) number accordingly.)
- write first holder’s name and pan details here. In case of investors is minor please name and pan details his guardian.
- if second and third holder’s provided by your investment please write name and pan details here
